Terms and Conditions
Effective Date: July 25, 2025
Welcome to Shoreside Wellness. By accessing our website or booking services with us, you agree to the following terms and conditions. Please read them carefully.
1. Use of Website
This website is for general informational purposes only. It does not provide medical advice, diagnosis, or treatment. Content is not intended to replace consultation with a regulated healthcare professional.
If you have medical concerns, please consult with a qualified provider. Use of this website or our services does not establish a practitioner-client relationship until a formal intake and consent process is completed.
2. Regulated Health Services
Our acupuncture, chiropractic, and registered massage therapy (RMT) services are delivered by professionals registered with the appropriate regulatory colleges in Ontario:
-
Acupuncturists – CTCMPAO
-
Chiropractors – CCO
-
RMTs – CMTO
These practitioners follow the standards of practice, professional conduct, and confidentiality requirements set by their respective colleges.
Relaxation massage services are provided by unregulated service providers (masseuses) and are not eligible for insurance reimbursement.
3. Appointments & Cancellations
-
Booking: Appointments can be made online, by phone, or in person.
-
Intake Forms: New clients must complete intake forms prior to their appointment. These are sent via email and must be submitted before treatment can begin.
-
Informed Consent: You will be asked to review and sign a consent form before receiving treatment. Consent can be withdrawn at any time.
-
Cancellations: We require 24 hours’ notice for cancellations or rescheduling. Late cancellations or missed appointments will be subject to the full fee.
-
Arrival: Please arrive 5–10 minutes early to allow time for check-in.
-
Late Arrivals: Sessions may be shortened if you arrive late to avoid delaying the next client.
4. Payment & Fees
Payment is due at the time of service. We accept debit, cash, and e-transfer only. Credit cards are not accepted.
Treatment fees vary by service and may change without notice. Receipts are issued for services provided by regulated health professionals and may be eligible for reimbursement under private health insurance.
5. Confidentiality & Record Keeping
All client records are maintained securely and in compliance with applicable privacy laws and the standards of the CMTO, CTCMPAO, and CCO. Your information will not be shared without your written consent, unless required by law.
6. Scope of Services
Each practitioner operates within their regulated scope of practice as defined by their college. If a condition is outside our scope, we may refer you to another appropriate healthcare provider.
Please inform us of any changes to your health history, medications, or conditions before each treatment.
7. No Guarantee of Outcomes
While our services are intended to support your wellness and recovery goals, individual results may vary. Shoreside Wellness does not guarantee any specific outcome or improvement.
